Markdown xss backport (#8244)

* Update helpers.py

* Update mixins.py

* format

* format

* Allow horizontal rule in markdown

* More instructive error msg

* Specify output_format to markdown.markdown

* Cleanup

* Adjust allowable markdown tags

* Add unit test for malicious markdown XSS
